Action Plan

Caring has been our way of life since 1842 and we provide a home for over 1,000 people across England and Wales – while supporting many more. Whether people need residential or nursing care, dementia support or limited sheltered accommodation, we care for them professionally and kindly. We treat people as individuals – and we work together with everyone to design a care plan that meets their needs and allows them to experience wellbeing and meaningfulness. Commitment 1 - Relating to:

Independence/Interdependence/Dependence - it will require sustained effort , to ensure our assessments and support focus on the assets of the individual to produce an enabling plan of care.

 

How will your organisation achieve Commitment 1?

 

Our homes work with individuals to understand what the person wishes for themselves and how we can come to shared decisions to safely support this.

  • We have the right to be recognised as who we are, to make choices about our lives including taking risks, and to contribute to society. Our diagnosis should not define us, nor should we be ashamed of it.

Commitment 2 - Relating to:

Community/Isolation - to ensure we focus on holistic care were all the individual needs are addressed

 

How will your organisation achieve Commitment 2?

 

We will work with individuals ,families, advocates , ,friends and the wider MDT to ensure we are an inclusive and reduce the risk of isolation. We have trained dementia mappers who work within the organisation who support the services in experiencing care from the residents perspectives.

  • We have the right to continue with day to day and family life, without discrimination or unfair cost, to be accepted and included in our communities and not live in isolation or loneliness.

Commitment 3 - Relating to:

Carers - our organisation values the training and competency of staff and extends to extra training and support .

 

How will your organisation achieve Commitment 3?

 

We are launching a new dementia training programme in 2019. We are also working closely with partnership organisations to look at dementia support.

  • We have the right to an early and accurate diagnosis, and to receive evidence based, appropriate, compassionate and properly funded care and treatment, from trained people who understand us and how dementia affects us. This must meet our needs, wherever we live.
