Action Plan

Memory Tracks is a ‘music-care platform’, a service that assists with the care for older individuals and particularly those living with dementia. Memory Tracks uses song-task association to help those living with cognitive issues recall activities, maintain routines, and remain more independent in the early stages of the illness. Later in the illness it supports care practitioners making care giving much easier. Music helps lower anxiety, depression and agitation, and as a result people can be easier to care for. Their Memory Tracks also serve as a connection between the person and their family or carer, assisting recognition and shared experiences.

Commitment 1 - Relating to:

We work with care homes to support the use of music in providing better and more sensitive care to their residents.

 

How will your organisation achieve Commitment 1?

 

In Spring 2019 we will launch our Memory Tracks app that can be used by care practitioners to reduce stress, improve recall, and reduce confusion in providing the daily care needs to their residents.

  • We have the right to an early and accurate diagnosis, and to receive evidence based, appropriate, compassionate and properly funded care and treatment, from trained people who understand us and how dementia affects us. This must meet our needs, wherever we live.

Commitment 2 - Relating to:

We will provide carers, whether within a care home environment, or at home, with the option to use personalised music in their daily care provision.

 

How will your organisation achieve Commitment 2?

  • We have the right to be respected, and recognised as partners in care, provided with education, support, services, and training which enables us to plan and make decisions about the future.

Commitment 3 - Relating to:

We will continue to carry out research into the impact of music in the provision of care to people living with dementia.

 

How will your organisation achieve Commitment 3?

 

We are already carrying out research and will endeavour to find more funding to continue this work.

  • We have the right to know about and decide if we want to be involved in research that looks at cause, cure and care for dementia and be supported to take part.
