Action Plan

Healthcare software that supports all those involved in the care journey from diagnosis to end of life by delivering engaging digital activities that generate a unique data set that in turn radically enhance person centred care .

Commitment 1 - Relating to:

We will continue to build our care in the community and community engagement tool that will support those cared for in the community and their family and carers, as well as providing tools that support the formal carers they engage with.

 

How will your organisation achieve Commitment 1?

 

We are working with Hospitals and local authorities to assist them use ReMe as part of their strategy for engagement with those cared for in the community, to reduce isolation, reliance on GP's and A&E and to put the person back at the centre of their care.

  • We have the right to continue with day to day and family life, without discrimination or unfair cost, to be accepted and included in our communities and not live in isolation or loneliness.

Commitment 2 - Relating to:

ReMe is being used in care homes, hospitals and day care centres and provides a means for carers to better know the person through the use of Electronic Life records created through delivered person care activities. Our work this year will focus on those cared for in the community to assist enhance their ability for self care management.

 

How will your organisation achieve Commitment 2?

 

We will work with as many organisations as wish to work with us to achieve enagement across the UK. And we will continue to build partnerships with those tech providers that can support our activities.

  • We have the right to an early and accurate diagnosis, and to receive evidence based, appropriate, compassionate and properly funded care and treatment, from trained people who understand us and how dementia affects us. This must meet our needs, wherever we live.

Commitment 3 - Relating to:

We will continue to improve our means of collecting data, so that it can be used for research purposes, since at last there appears to be a genuine policy and budget shift towards recognising that investing in care is as important as cure.

 

How will your organisation achieve Commitment 3?

 

ReMe enables the consumer opted in and GDPR compliant collection of life date (our Electronic Life Records data set) that can enable a better understanding of what is required to prevent dementia through learning from those with dementia today about their likes, dislikes, habits, preferences, life stories, memories, etc. This data set will be made available to our partners once we have improved our data collation processes.

  • We have the right to know about and decide if we want to be involved in research that looks at cause, cure and care for dementia and be supported to take part.

Commitment 4 - Relating to:

Post the launch of the ReMe care at home app that includes an integration with our proprietary Alexa care skills and pending inclusion in the Emis GP app library enabling GP prescribing of ReMe as a post diagnosis support tool, ReMe is now globally downloadable.

 

How will your organisation achieve Commitment 4?

 

Being a member enables us to be part of a key care ecosystem of care and innovation and provides us with support for what we are doing, since we are unfunded and unsupported in our mission and so need all the help we can get.
